Wrong mindset. People are stuck in old assumptions about how things work. The organisation's habits and values no longer match its situation. The answer is transformation.
Institutional Authority
No mechanism for prohibition. Everyone already knows what needs to stop. No one has the standing, the mandate, or the institutional backing to say it. The answer is building that capacity.

Process absorbs the energy that would otherwise confront the problem. The governance becomes the activity. Organisations can add process indefinitely without ever acting.
Better Frameworks
Understanding the system is not the same as having authority to change it. The sophistication of the model can become a permanent substitute for the confrontation it was supposed to enable.
Structural Redesign
Restructuring is disruptive and slow. Organisations can be in permanent reorganisation—the restructuring itself becomes displacement activity, and the original problem migrates to the new structure.
Cultural Change
Culture change is unmeasurable and unfalsifiable. It can absorb unlimited time and investment without ever producing the specific confrontation the organisation was avoiding in the first place.
Institutional Authority
Requires governance architecture that may not exist. If no one currently has the standing to prohibit, this approach names the gap clearly but offers no guaranteed route to filling it.
